Your First Appointment

Our cardiologists work with your primary care provider to ensure that there is a continuum of care for your medical condition. In order to provide personalized, quality medical care, we must know as much as possible about your current and past medical history. You will be asked to complete patient registration forms prior to your visit.

Please arrive a minimum of 15 minutes prior to your appointment to complete all necessary paperwork. Your provider will also spend time asking about your medical history. If you are taking medications, please bring them with you. Please bring any lab results received within the last month to your appointment. Your first visit will last approximately one hour.

Please fill out the Patient History Assessment Form ahead of time and fax back to our office at 269-373-6270. If you do not have access to a fax machine please mail back to our office one week before your scheduled appointment.
